Three non-continuous ECG strips show episodes of polymorphic ventricular tachycardia occurring in a middle-aged man who presented with syncope. Of note is the extreme HR during the VT (up to 390bpm) and its morphological similarity to torsades de pointes (‘twisting of the points’); the latter arrhythmia however is associated with a prolonged QT interval. Polymorphic VT is an extremely dangerous arrhythmia and requires implantation of a cardioverter defibrillator device.
