The patient was an 85 year old gentleman referred for Holter monitoring because of falls. The top ECG strip shows Wenckebach at 0059 hours during sleep followed (middle strip) by a brief (4.8 second) run of PAT with high degree AV block reverting to sinus rhythm with 1st degree AV block and a very long PR interval (0.48 seconds).
